  • 摘要: 复杂性创面的修复已从单纯闭合创面转向功能性重建,但目前对其定义、修复理念和未来方向还缺乏系统阐述。在文献回顾和综合分析的基础上,该文提出复杂性创面的界定应综合考虑“愈合时序异常、解剖结构受累、危险因素并存及管理需求复杂”4个维度。可将该类创面功能性修复的核心理念总结为4个阶梯式的原则:供区与受区的生物学匹配、供区损伤最小化、恢复外观与感觉功能,以及深层结构,包括血管、神经、肌腱、骨骼与关节的功能性重建。该文系统梳理复杂性创面修复从皮肤移植奠定基础,到显微外科功能再造,再到多学科融合探索再生的发展历程,重点阐述中国学者在该领域做出的原创贡献。未来,再生医学、智能导航、超级显微外科及精准分型等技术深度融合,将进一步促进复杂性创面修复从解剖修复向功能再生发展。
