摘要: 肌肉减少症是威胁老年人健康的核心问题。老年难愈性创面患者发生肌肉减少症的风险更高,预后更差,但临床对其识别和管理仍相对不足。从业者在认识两者双向恶化机制的基础上,应借鉴相关领域共识和指南,将肌肉减少症纳入此类患者的常规筛查、综合评估与动态复评,并从营养支持与代谢调节、免疫与炎症调控、创面处理、体力活动与康复治疗及心理情绪支持等方面实施综合管理。未来可借助人工智能和多组学等技术,通过更多研究与临床实践建立老年难愈性创面患者肌肉减少症的筛查、诊断与治疗共识,以改善其临床结局和生活质量。

Abstract: Sarcopenia is a major threat to the health of older patients. Older patients with refractory wounds are at increased risk of sarcopenia and have poorer outcomes, however, its clinical recognition and management remain inadequate. Based on an understanding of the bidirectional deterioration between sarcopenia and refractory wounds, healthcare professionals should draw on relevant consensus statements and guidelines, incorporate sarcopenia into routine screening, comprehensive assessment, and dynamic re-assessment, and provide integrated management encompassing nutritional support and metabolic regulation, immune and inflammatory modulation, wound care, physical activity and rehabilitation, and psychological support. Future research and clinical practice, supported by artificial intelligence and multi-omics, will be conducted to establish consensus on screening, diagnosis, and treatment for sarcopenia in older patients with refractory wounds, thereby improving their clinical outcomes and quality of life.
